Documents to submit are as follows:

• An application form to Jichi Medical University (Please use the form JMU requires.)
• A questionnaire (Please use the form JMU requires. When available, attach a paper or papers written by the applicants as author or co-author.)
• One recommendation letter (A letter from President or Dean who made agreement at the school
  of the applicant belongs or belonged to.)
• An official document about 2. Requirements (6) of Application Guideline
• Score sheet of TOEIC, TOEFL or IELTS (copy is acceptable)
  *Score sheet should be issued within 4 years of the month that examination is held.
• A health certificate (Please use the form JMU requires.)
• A diploma issued by the medical school and graduate school the applicant graduated from.
• An academic record issued by the medical school and graduate school the applicant graduated from.

[Notes] All these required documents must be completed in either Japanese or English. Documents in other languages are accepted only when they are accompanied with a Japanese or English translation by a public institution for translating.

These documents should be submitted to the applicant’s university office which manages this program.
